After the Fourth Impact, Shinji, Asuka, and Rei are left without their Evangelions on Earth, which has been largely destroyed. They seek refuge in one of the few remaining human settlements. Despite living very different lives from their time as pilots, they know that danger still threatens the world. Another impact is approaching-one that will determine the fate of humanity. Meanwhile, the Human Instrumentality Project is initiated, and WILLE makes a final effort to stop it from causing the Final Impact.

The film was released internationally on August 13, 2021 via the Amazon Prime Video streaming service. The film was originally announced alongside Evangelion: 3.0 You Can (Not) Redo in 2008 and was slated for a 2015 release, but was delayed several times due to various production issues and the COVID-19 pandemic.The film has been dubbed in 10 languages: Chinese, English, French, German, Hindi, Italian, Korean, Brazilian Portuguese, Latin American Spanish, and Peninsular Spanish. Evangelion: 3.0+1.0 Thrice Upon a Time won the Animation of the Year award in the Film category at the Tokyo Anime Award Festival in 2022.
