Dr. Kenzou Tenma is a skilled neurosurgeon working at Eisler Memorial Hospital in Düsseldorf. Initially, his exceptional surgical abilities earn him favor from the hospital director. However, after experiencing a moral dilemma, he chooses to save a young boy's life instead of the city mayor, leading to his unjust demotion. Later, when the director and several prominent doctors are murdered, Dr. Tenma becomes a suspect in the killings. Nine years later, he discovers that the perpetrator is none other than the boy he saved, Johan Liebert. Plagued by guilt, he resolves to find and end the life of this "monster" he feels responsible for creating.
