Kenzou Tenma was a famous Japanese neurosurgeon who worked in post-war Germany. He faced a tough decision when he had to choose between operating on an orphan boy, Johan Liebert, who was near death, and the mayor of Düsseldorf. Despite the risks, Tenma decided to save Johan, which cost him his reputation and even his fiancée's love. Later, after Heinemann, the hospital director, was killed, Tenma got a chance to redeem himself by becoming the head of the surgical department and being close to becoming the director himself. However, things took a dark turn when he became involved in a series of gruesome murders that had been happening throughout Germany. The killer was none other than the same monster that Tenma had saved nine years ago.
